As the lawful parent or guardian, I give permission for As You Are Dance Movement AYA) to use images or videos of my child for promotion, inlcuding but not limited to AYA's website or social media.

As the lawful parent or guardian, I agree to release and hold harmless AYA, including its owners and all personnel, from any and all responsibility, claims, or legal actions that may arise from any injury, loss, or harm—including but not limited to illness or death—sustained by the participant and/or myself while on the premises of AYA, at any location under its direction, or during travel to and from any such location or event connected with its programs.

As the lawful parent or guardian, I understand that if my child participates in the 2nd grade dance routine for the talent show, they are required to wear the provided As You Are Dance logo shirt during the performance. The shirts must be consolidated and returned by one parent during the week following the performance.

By signing below, I acknowledge that I have read, understood, and agree to the terms stated above.
